LOUISVILLE YOUTH SPORTS GROUP IMPORTANT FORMS:

ALL PARTICIPANTS MUST SIGN THE OFFICIAL LYSG RELEASE/WAIVER IN ORDER TO PARTICIPATE IN ANY LYSG EVENTS. PLEASE PRINT AND BRING SIGNED WAIVER WITH YOU. WAIVERS WILL BE AVAILABLE AT THE EVENT AS WELL TO BE SIGNED BEFORE STARTING ACTIVITY.
*** Please note Spring regular season 7 on 7 make up games will be played on Sunday afternoons.
Admission to our 7 on 7 games will be $5 for Adults and 17 & under free. (Admission Fees help to offset the cost of field, referee and security expenses.)
*** Concessions will also be available at the games serving all your favorite food, drinks and snacks.
In the event of inclement weather cancellations or delays immediate updates will be posted on our Louisville Youth Sports Group Social media sites. Please follow, like and share.

*** All Covid 19 CDC Guidelines will be in place and enforced. Face Mask and social distancing may be required for spectators and players. Thanks for help with helping us keep the game safe for all our players, coaches, staff and spectators.
